Associated Allergists and Asthma Specialists, Ltd., is dedicated to providing diagnosis and treatment of allergies and asthma in greater Chicago. Appointments are available at our offices in Orland Park, Naperville, Hoffman Estates, Flossmoor, Highland Park, Lincoln Park, Arlington Heights, Plainfield, and Barrington, Illinois.

What to Bring to Your Appointment

  • Your insurance card
  • Physician referral forms, if required by insurance
  • List of prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about your medical and surgical history
  • Recent x-rays or relevant records
  • New patient form

Insurance and Payment Information

Associated Allergists and Asthma Specialists, Ltd., is a provider for Medicare and most major insurance plans, and provides insurance billing. You are responsible for charges that are not covered by your insurance, at the time of your appointment.

Your insurance company may require a co-payment at the time of your appointment. When necessary, our staff will work with patients to set up a payment plan.

What Is An Allergist?

An allergist is a physician trained to determine the cause of allergies, whether they are related to or caused by foods, environmental factors (such as pollen), drugs, or topical substances. Conditions that an allergist commonly treats include the following:
